Subcontracting Forms

If you are a subcontractor or vendor that wants to get on our bidders list there are several forms that need to be filled out and returned to us for our files and records keeping. The forms and their purposes are listed below.

Environmental Requirements (General)

To facilitate expeditious processing of Subcontractor invoices, all Subcontractors should adopt the enclosed procedures.

Insurance Requirements

Subcontractors or vendors that may provide services at our facility or work onboard ships must meet our insurance requirements.

Health and Safety Requirements for Non-Continental Maritime Employees

Applies to all Continental Maritime contractors, subcontractors, third parties, vendors, visitors, and any other non-employee of Continental Maritime. Contractor Pre-Performance Questionnaire

Socio-Economic Survey

Subcontractors and vendors are required to fill out this form. The information provided helps us to categorize your company for the many reports we must turn into the government concerning small business subcontracting, which includes those that only provide materials. Once on the bidders list you may be asked to fill out this form annually so that any changes can be documented. For Small Disadvantaged Businesses or SDB's you must be certified through the Small Business Administration (SBA). You can no longer self certify as an SDB. Technical Manual Request Form

Terms and Conditions (T&C's)

This is primarily for those subcontractors or vendors that provide a service. T&C's must be signed and returned.
